3. Choose the possible qualification path
Select SNAP, Medicaid, SSI, Federal Public Housing Assistance, Veterans Pension or Survivors Benefit, a qualifying Tribal program, household income, or not sure. The selection records a possible path and is not an eligibility finding.
4. Add household size and device preference
Enter household size and choose a device category preference. A preference does not reserve inventory, guarantee a model or establish a promotional price.
5. Review and consent
Review the complete summary and the focused acknowledgments. When secure intake is active, the final security check protects the real submission. When intake is not active, the page clearly states that nothing was submitted.
No public document uploads
- No Social Security number or last four digits.
- No EBT card number or PIN.
- No Medicaid member ID or other benefit-account ID.
- No bank or payment-card information.
- No passwords.
- No identity photo, document photo, camera capture or file upload.
After a successful receipt
A successful online submission returns a non-sequential GT reference. Keep it for records and support. The reference confirms receipt only. It is not an approval number and is not sufficient authentication for application status.

- Staff request minimum necessary proof separately only if it is needed.
- The applicable eligibility process verifies qualification.
- Company review, service availability and any actual device offer are evaluated separately.
- Fulfillment or activation follows only when the real customer record supports it.
Secure status for returning applicants
When online status verification is active, use the GT reference and the same application email to request a one-time verification code. Status is shown only after that code is verified. If the service is unavailable, the page returns no code or status.
